Adding to Nakao’s comments, Hirayama explained that he feels “fairness between the player and enemy is crucial” and that “whether or not death feels like it was your fault is a huge part of maintaining that sense of fairness.”

The developers do not want to punish players, but to help them “reflect” on each death and think about what “they could have done differently.”
